• Blue Choice PPO
• Blue Choice Options PPO

A monthly superbill/statement is provided upon request if clients wish to submit to their out-of-network insurance. Kender Family Services does not submit claims to out-of-network insurance, and this remains the responsibility of clients.

Payment Information:

All clients have the option of paying out-of-pocket, and payment is required the day of service. The payment scale is based on individual and treatment factors and currently ranges from $200-$350.

Cash, all major credit cards, and Quick Pay via Zelle are accepted.
